A correction was made at 6:35 p.m. Dec. 12, 2023: Oregon State Police issued a correction on the date of a fatal crash that occurred on I-5 near Wolf Creek. The crash took place on Dec. 8.

FRIDAY, DEC. 1

• 10:04 a.m. — Police responded to a report of a suspicious person in the 1700 block of Main Street. A man was contacted and warned for going through other people’s trash.

Other calls: Police responses included contact with a woman and child at 1:34 p.m. (Philomath City Park), dog at large at 2:22 p.m. (Main Street), welfare check at 2:48 p.m. (Applegate Street) and a traffic hazard at 7:23 p.m. (Main Street and Green Street). In addition, police assisted the Corvallis Police Department on a call at 1 a.m. (Southwest Philomath Boulevard, Corvallis). PPD reported 14 traffic stops.

SATURDAY, DEC. 2

• 4:39 p.m. — Police responded to a report of theft in the 600 block of Main Street. Police took a report about the theft of wood and assigned a case number.

• 5:01 p.m. — Police took a report from a Main Street location of juveniles missing. The juveniles later returned home.

• 9:42 p.m. — Police arrested a 47-year-old Philomath man on a misdemeanor charge of driving under the influence of intoxicants (unclassified) during a traffic stop at Grange Hall Road and Fern Road. The man was issued a citation in lieu of custody.

Other calls: Police responses included a report of a domestic disturbance at 1:34 a.m. (Main Street), event detail at 7:34 a.m. (Northwest Kings Boulevard, Corvallis), security check at 8:42 a.m. (Main Street) and a noise disturbance at 11:16 p.m. (Applegate Street and South 11th Street). PPD reported 17 traffic stops.

SUNDAY, DEC. 3

• No calls of significance were reported.

Other calls: Police responses included a welfare check at 12:56 a.m. (McCall Way), suspicious circumstance at 3:16 a.m. (North 11th Street), security check at 2:29 p.m. (North 11th Street), security check at 3:15 p.m. (Applegate Street) and a noise disturbance at 9:55 p.m. (Adelaide Drive).  In addition, police assisted the Benton County Sheriff’s Office at 10:36 a.m. at the scene of a crash (Highway 20 and Priest Road) and the Corvallis Police Department on a call at 11:02 a.m. (Southwest Research Way, Corvallis). PPD reported six traffic stops.

MONDAY, DEC. 4

• No calls of significance were reported.

Other calls: Police responses included a suicidal subject at 3:33 a.m. and phone harassment at 4:35 p.m. (South 16th Street). In addition, police assisted the Corvallis Police Department on a call at 2:55 p.m. (Southwest Philomath Boulevard and Southwest 71st Street, Corvallis). PPD reported one traffic stop.

TUESDAY, DEC. 5

• 10:03 a.m. — Police took a report of threats in the 2000 block of Chapel Drive. No other details were made available other than a report was taken and assigned a case number.

• 2:21 p.m. — Police took a report of fraud in the 2400 block of Newgate Drive. No other details were made available other than a report was taken and assigned a case number.

Other calls: Police responses included criminal mischief at 7:32 a.m. (North 16th Lane), non-911 issue at 8:21 a.m. (Main Street), harassment at 1:21 p.m. (Pioneer Street and North 12th Street), trespass at 3:14 p.m. (Main Street), criminal mischief at 5:41 p.m. (Applegate Street) and a suspicious circumstance at 8:39 p.m. (North 11th Street). In addition, police assisted another agency on a call at Main Street and North 18th Street. PPD reported two traffic stops.

WEDNESDAY, DEC. 6

• 1:22 p.m. — Police took a report from a man who lost his wallet, which contained identification and credit cards, at a Philomath business.

Other calls: Police responses included a repossessed vehicle at 7:07 a.m. (North 18th Street), person assist at 10:53 a.m. (South 19th Street and Chapel Drive) and domestic disturbance at 5:48 p.m. (South 13th Street) and found a dog at 6:24 p.m. (Dove Place). In addition, police assisted another agency on a call at 1:39 p.m. (Main Street). PPD reported two traffic stops.

THURSDAY, DEC. 7

• 2:30 a.m. — Police responded to a report of assault at a North Ninth Street location. No other details were made available other than a report was taken and assigned a case number.

• 9:59 a.m. — Police reported a call that included a possible driving while suspended violation at 10th Street and Main Street. No other details were made available other than a report was taken and assigned a case number.

• 1:44 p.m. — Police reportedly executed a warrant in the 1000 block of Applegate Street. Details on a possible arrest were not immediately available.

• 5:05 p.m. — Police reported a call that included a possible driving while under the influence of intoxicants in the 1300 block of Main Street. No other details were made available other than a report was taken and assigned a case number.

• 7:14 p.m. — Police reportedly executed a warrant in the 1300 block of Main Street. Details on a possible arrest were not immediately available.

Other calls: Police responses included an animal complaint at 8:18 a.m. (North Ninth Street and College Street) and a welfare check at 8:20 p.m. (Pioneer Street). PPD reported five traffic stops.

FRIDAY, NOV. 24

• 4:16 p.m. — Police arrested a 53-year-old Philomath man in the 1400 block of Main Street on a warrant out of Linn County. The man was transported to Linn County Jail.

TUESDAY, NOV. 28

• 10:37 p.m. — Police arrested a 33-year-old Philomath man on a warrant related to a misdemeanor charge of unlawful dissemination of an intimate image.

TUESDAY, NOV. 21

• 7:11 p.m. — Deputies responded to a two-vehicle crash on Highway 20 near Shrike Way involving a vehicle that had fallen off a flatbed trailer and crashed into a car that was following. No citations were issued.

SATURDAY, NOV. 25

• 7:26 a.m. — Deputies responded to a single-vehicle crash on Highway 20 about 2 miles east of Blodgett involving a 43-year-old Woodburn man who lost control of the Nissan Pathfinder he was driving on an icy road, crossed the centerline, overcorrected and crashed into a ditch. No injuries were reported. A fire broke out in the vehicle’s engine compartment, which was extinguished by Blodgett-Summit Rural Fire.

SUNDAY, NOV. 26

• 6:16 p.m. — Deputies responded to a single-vehicle crash on Maxfield Creek Road about 4 miles east of its intersection with Kings Valley Highway. The driver, a 41-year-old Monmouth woman, was driving a Pontiac Vibe that slid off the road and into a tree on the other side of a ditch. The woman had possible injuries and planned to seek medical attention the following day for herself and daughter, who was a passenger. See separate story.

FRIDAY, DEC. 1

• 10:53 a.m. — Deputies responded to a single-vehicle crash on Highway 20 near Lone Star Road. The driver, a 44-year-old Newport woman, was transported to Good Samaritan Regional Medical Center. See separate story.

MONDAY, DEC. 4

• 9:31 a.m. — Deputies responded to a report of a pitbull that was in the garage of a residence located along Highway 20 about 1-1/2 miles east of Blodgett. The dog was transported to Heartland Humane Society in Corvallis.

FATAL CRASHES

FRIDAY, DEC. 8

• 6:57 a.m. — Troopers responded to a single-vehicle crash on Interstate 84 East about 8 miles south of Baker City that took the life of Michael Workman, 65, of Montpelier, Idaho. OSP’s preliminary investigation indicated that Workman was driving a Toyota Tacoma that left the roadway and rolled several times and traveled approximately 320 feet before stopping. Workman, who was not wearing a seatbelt, was declared deceased at the scene.

• 5:54 p.m. — Troopers responded to a crash on Interstate 5 about 1 mile south of Wolf Creek that took the life of Tami Lee, 62, of Medford. OSP’s preliminary investigation indicated that Lee was driving a Kia south on I-5 and is believed to have struck an animal. She apparently exited the vehicle to check on the animal and was struck in the slow lane by a semi-truck driven by a 65-year-old Elmira man. Lee was pronounced deceased at the scene. The truck driver remained at the scene to cooperate with the investigation.
