MONDAY, MAY 4
• 8:21 a.m. — Emergency medical services personnel were dispatched to a crash at Chapel Drive and Bellfountain Road and responded with seven personnel in an engine and two command vehicles. Upon arrival, crews found two vehicles involved in the crash with one coming to rest in the ditch after the collision. No injuries were reported. Crews cleaned debris from the road.
• 9:31 p.m. — Firefighters responded to a fire alarm activation in the 500 block of South 19th Street.

TUESDAY, MAY 5
• 8:30 a.m. — Firefighters responded to a fire alarm activation in the 1800 block of Bullevard Street.
• 6:16 p.m. — Firefighters were dispatched to provide mutual aid for Corvallis Fire Department at its Station 2 while crews were obligated on an incident. The Philomath crew responded with three personnel in an engine.
• 7:18 p.m. — Firefighters responded to a fire alarm activation in the 700 block of South 15th Street.
Other calls: Emergency medical services responded to calls at 7:27 a.m. (Pioneer Street), 4:39 p.m. (Worth Way) and 6:20 p.m. (Hollyhock Circle).
WEDNESDAY, MAY 6
• 9:53 a.m. — Firefighters were dispatched to a smoke investigation near Harris Road and responded with seven personnel in an engine, a brush truck and a command vehicle. Upon arrival, crews determined that the smoke was from a fuels reduction burn near Blodgett.

THURSDAY, MAY 7
• 4:43 p.m. — Emergency medical services personnel were dispatched to a mutual aid fall injury at Beazell Memorial Forest and responded with six personnel in a rescue vehicle and two command vehicles. The Philomath crew assisted Hoskins-Kings Valley Rural Fire Protection District with the rescue of a hiker approximately a mile from the trailhead. The patient was packed down to the parking area for transport by a Corvallis Fire Department ambulance.
• 11:31 p.m. — Firefighters responded to a fire alarm activation in the 400 block of South 13th Street.
