TUESDAY, JUNE 17
• 3:41 p.m. — Troopers responded to a four-vehicle crash on Highway 99 about 3 miles north of Cottage Grove that took the life of Joseph M. Peters, 35, of Cottage Grove. OSP’s preliminary investigation indicated that a Chevrolet Silverado dually pickup driven by a 46-year-old Creswell man was heading south on Highway 99 when it slowed to negotiate a turn. Behind the Silverado was a Chevrolet Tahoe driven by a 35-year-old Cottage Grove woman and that vehicle was rear-ended by a Mazda Tribute driven by Peters. The Mazda then veered into the northbound lane and collided with a Ford F-350 pickup, driven by a 34-year-old Eugene man, that was towing a dump trailer loaded with dirt and rocks. Peters was transported to an area hospital and later died from his injuries on June 20. The driver, plus a 14-year-old girl and a 9-year-old girl, in the Tahoe were transported to the hospital with non-life-threatening injuries. The F-350’s driver reported minor injuries and was not transported. The Silverado’s driver was not injured.
FRIDAY, JUNE 20
• 4:06 p.m. — Troopers responded to a single-vehicle crash on Highway 26 about 11 miles south and east of Seaside that took the lives of Richard Scott McIntosh, 74, and Michele Arnold McIntosh, 72, both of Seaside. OSP’s preliminary investigation indicated that Richard McIntosh was driving a Genesis GV70 west on Highway 26 when the vehicle swerved onto the shoulder, traveled up a dirt embankment and crashed into a tree. He was pronounced deceased at the scene. Michele McIntosh was transported to an area hospital and later died from injuries sustained in the crash.
• 8:54 p.m. — Troopers responded to a single-vehicle crash on Highway 47 near Banks that took the life of Jeannine Bradvice, 72, of Banks. OSP’s preliminary investigation indicated that a Subaru Forester driven by Bradvice was headed north on Highway 47 when she lost control of the vehicle, veered into a ditch on the west side of the highway and struck a tree. Bradvice was pronounced deceased at the scene. OSP reported that speed was believed to be a contributing factor in the crash.
THURSDAY, JUNE 26
• 1:21 p.m. — Troopers responded to a three-vehicle crash on Highway 101 in Clatsop County about 2 miles south of its intersection with Highway 26 that took the lives of a 15-year-old girl and a 12-year-old girl. OSP’s preliminary investigation indicated that a Toyota Tacoma driven by a 30-year-old Seaside man was headed south on Highway 101 when for unknown reasons the vehicle crossed into the northbound lane and struck head-on a Honda CRV driven by a 76-year-old San Francisco, California, woman. The Honda was pushed to the northbound shoulder while the Toyota continued in the northbound lane and struck head-on a Nissan Rogue driven by a 57-year-old Albuquerque, New Mexico, man. The 15-year-old girl and the 12-year-old girl were passengers in the Nissan and declared deceased at the scene. The driver of the Nissan and another passenger, a 52-year-old Albuquerque woman, suffered serious injuries and were transported to an area hospital. The drivers of the Toyota and Honda also suffered serious injuries and were transported to the hospital.
