SATURDAY, SEPT. 7

• 1:08 a.m. — Troopers responded to a two-vehicle crash on Highway 6 south of Banks that took the life of Leonardo Franco-Velazquez, 22, of Gold Bar, Washington. OSP’s preliminary investigation indicated that Franco-Velazquez was driving west in a GMC Sonoma when for unknown reasons, the vehicle crossed into the oncoming lane and struck head-on a Honda Accord driven by a 65-year-old North Plains man. Franco-Velazquez was declared deceased at the scene. A passenger in the GMC, a 20-year-old man from Tukwila, Washington, and the driver of the Honda, were both critically injured and transported to the hospital.

MONDAY, SEPT. 9

• 6:07 a.m. — Troopers responded to a two-vehicle crash on Highway 138E about 1 mile east of Glide that took the life of Thomas Lee Bailey, 38 of Idleyld Park. OSP’s preliminary investigation indicated that Bailey was headed west on the highway when a Jeep Cherokee, driven by a 66-year-old Roseburg man in the opposite direction, attempted a left turn in front of the motorcycle. The vehicles collided in the westbound lane. Bailey was declared deceased at the scene. The Jeep’s driver was not injured and cooperated with the investigation.
