A 65-year-old Prineville woman died Thursday in a head-on collision on Highway 99W and Northwest Elks Avenue in Corvallis, law enforcement officials reported.

Corvallis Police and Corvallis Fire received a call about a crash at the 99W-Elks Avenue intersection at 5:38 p.m. July 6. Police determined that a Subaru Legacy traveling south on Highway 99W crossed the center lane line causing a head-on collision with a Hyundai Elantra traveling north.

The Subaru’s driver and lone occupant, a 74-year-old Monmouth woman, was transported to Good Samaritan Regional Medical Center. The Hyundai had four occupants that included the woman who was declared deceased at the scene. The driver, a 32-year-old Albany woman, and two other passengers, a 37-year-old Albany woman and a juvenile were transported to the hospital.

A third vehicle involved in the crash had minor damage with no injuries to its driver or passenger.

Corvallis Police and Benton County Sheriff’s Office Major Accident Investigation Team responded to the scene.

The highway was closed for approximately 4-1/2 hours. Corvallis Police was assisted at the scene by BCSO, Corvallis Fire and the Oregon Department of Transportation.
