Philomath-area high school and middle school athletes are invited to take advantage of an opportunity to receive a free sports physical from the Corvallis Clinic. The physicals will run from 1-4 p.m. Friday in the Philomath High School auditorium.
The wearing of masks and social distancing will be required for the event.
The Corvallis Clinic Orthopedics and Sports Medicine Department is sponsoring the free sports physicals in partnership with the Philomath High School Booster Club.
Students planning to get a physical are required to have a parent or guardian fill out an examination consent form, which is available at this link.
In addition, the student or parent/guardian must submit the Oregon School Activities Association physical examination form, which includes a section about a student-athlete’s medical history. The form is available at this link.
